Telehealth Home Patient Monitoring

Telehealth monitoring supplements home care visits with assessment and education to a home care patient through the daily use of specialized equipment that can measure:

Blood pressure
Heart rate
Blood glucose levels
Pulse oximetry
Weight
Temperature

Answers to health questions about:

Diet
Sleep
Adherence to medication regiment
Symptoms management
Activity levels
When to follow up with physician

VNA patients who qualify for telehealth have had recent hospitalizations or emergency department visits, or need frequent monitoring.  Possible appropriate diagnoses include diabetes, CHF, pulmonary or cardiac conditions.  In most cases, when a patient is receiving covered VNA services, there is no extra charge for these services.  Please call (518) 489-1430 for more information.
