Resident of Albany, Saratoga or Rensselaer counties
Medically eligible for care in a residential health care facility
Medicaid eligible or private pay
Physician approved
Health care needs can be adequately met in home
Approval for services by local Department of Social Services for Medicaid patients
Availability of openings in the program
