Life Stability Programs
Community Mental Health and MSW Services
Patients may benefit from VNA services if they present with any of the following:
Psychiatric diagnosis (paranoid schizophrenia, acute depression, Alzheimer’s, dementia)
Hallucinations/delusions
Fears/anxiety/phobias
Substance abuse, self-destructive behaviors
Recent changes in memory, alertness, orientation, personality
Psychotropic medications
Crisis intervention as needed
Problems that limit effective treatment, rate of recovery, optimum health level
Patients may need help with long-term planning and access to other community services
VNA of Albany staff can assist providers and patients to define Medicare homebound status for potential Community Mental Health Services patients.
Personal Assistive Devices
Referrals can be made to arrange the following:
24-hour personal emergency response systems (monthly rentals)
Personal medication dispensing system, built-in monitoring capability (monthly rentals)
