Our program goal is to reduce and prevent falls that result in injury.
94% of patients completing this program had fewer falls than they did
60 days prior to receiving VNA services.

Common Risks:
History of at least one fall within the past year, or a chronic fear of falling
Difficulty walking, getting up or sitting down
Need to improve endurance, strength, fitness and balance
Sudden change in blood pressure
Improper footwear
Need for medication review, including evaluation for side effects
Need for assisted devices in the home
