Getting Started With the VNA of Albany


If You Are a Health Care Professional…

You can refer patients who reside in Albany, Saratoga and Rensselaer counties to the VNA of Albany by calling our direct patient acceptance line at (518) 489-1430, Monday through Friday, 8 am to 4 pm. 

For weekend, holiday or weekday referrals after office hours, please phone information to (518) 489-2681.

Or you can fax a VNA Fax Referral Form to (518) 489-2532 at any time.  Forms are available from our office. NOTE: This form must be signed by the patient’s referring physician.

If you need VNA Services for Yourself or a Loved One…

  • The patient must live in Albany, Saratoga or Rensselaer counties.
  • A physician must authorize VNA home care services.
  • You should be aware of insurance carrier eligibility requirements, such as need criteria for skilled services, and definitions of homebound status and part-time, intermittent care.
  • The VNA must be able to provide services safely in the home.

Since Medicare and Medicaid, and most private insurances require physician authorization for certified home health services, it is usually best to start with a conversation with the patient’s physician to ask if s/he feels that the patient is eligible and would benefit from home care. The patient’s physician can then contact the VNA to request services.

The patient can also call the VNA directly at (518) 489-1430 to get more information.

The VNA provides certified home health care services to all eligible patients in the counties of Albany, Saratoga and Rensselaer, and does not discriminate on the basis of race, culture or creed.  Please ask about our non-English speaking patient services.
