Experience in Discharge Planning & knowledge of Community Services is preferred.

Director of Patient Services

The Visiting Nurses of Albany, Inc. seeks an experienced and innovative home care leader for the position of Director of Patient Services. 

Reporting to the Chief Executive Officer, this person will direct the development, implementation, and oversight of clinical  programs to promote excellence in clinical practice and patient outcomes. 

We desire a creative and energetic leader to join our senior leadership team and help design and implement strategic plans to advance VNA programs, services, and innovative models of care. 

The VNA of Albany is certified in Albany, Rensselaer, and Saratoga counties, and has a 130-year reputation of service in the Capital Region. Programs and services include certified home care, long term home health care, infusion, cardiac and diabetes. 

Qualifications include BSN-Masters degree perferred in nursing, business or a health related field.  Minimum of 5 years supervisory/management experience, 2 of which should be in a community health setting.  We offer a competitive compensation package.  Relocation expenses are included.
